North London Forensic Service is part of Barnet Enfield and Haringey Mental Health Trust and provides care and treatment in conditions of low and medium security for those with mental illness or learning disability, many of whom have come into contact with the criminal justice system. Because our patients are with us for a long time they need access to a full physical health service and this was recognised by commissioners who have awarded significant new investment to expand our physical health team. If successful, you will play a key role in delivery a new and innovative physical health service, with the aim of: • Supporting service users to gain better control over their own health and more personalised care when they need it. • Reducing health inequalities • Boosting ‘out-of-hospital’ care, and bridging the gap between our inpatient and acute hospitals, as well as community services • Reducing pressure on emergency hospital services Key elements of our work will be: • Early detection, support and management of long-term conditions • Annual health checks/screening programmes • Health promotion • Healthy lifestyles • Physical activities / social prescribing If you have recently applied for the role and were not successful, please consider re-applying within six months time. 1. Provide high quality care to patients with both physical and mental health difficulties 2. Independently manage a caseload of patients who may have long term or complex multiple physical health needs 3. Support the delivery of physical healthcare training in collaboration with the head of physical health team 4. Take charge on a regular basis and assist in the supervision and/or development of junior staff, nurses, support workers and students, as well as support the development of evidence-based practice. 5.
